The Clinical Information Systems/Medical Record Department keeps patient information for both inpatient and outpatient services provided at Franciscan Children’s. The office is open Monday through Friday, 8 a.m. to 5 p.m.

Protected Health Information and Your Privacy

Protected Health Information (PHI) is information about your child’s health care that may include information that can identify your child, or is related to your child’s health, the care received here, or payment for care.Your child’s PHI may be used for or disclose treatment, payment, or health care operations.

You may request copies of your child’s health information, or request a list of people or organizations that have received information from us. You may also request how and where we communicate with you.

The Minimum Necessary Standard applies to any requests, uses, or disclosures of information for payment or health care operations. This means that the amount of information used, disclosed, or requested must be the minimum amount necessary for a defined purpose.

Access to Medical Information (Patient Portal)

The Patient Portal offers patients and families a secure online website that gives convenient 24-hour access to personal health information and medical records from anywhere with a secure internet connection.  Currently the Portal is open to families with children being seen in the Inpatient Rehabilitation program and the Inpatient Mental Health program.  For more information on the Portal, click here.

Request Copies of Medical Record Information

The privacy of your child’s health information is very important to us, and we make every effort to ensure that it is kept confidential. We are committed to respecting and protecting the rights of our patients and families.

You must submit written permission before we can release your child’s health information. To do this, please complete, sign, and mail or fax us the Release of Information form. You may download the form by clicking below, or call 617-254-3800, x1970 to request a form be sent to you.

Charges for Copies

There is no charge for copies requested by health care providers, or those needed for consultation or continuing care.

Copies requested for personal records, attorneys, insurers, etc., are charged according to the number of pages in the record. You will be pre-billed for the copies as soon as the number of pages is determined. Copies will be sent out upon receipt of the check or money order. If payment has not been received after 45 days, the request will be considered rescinded and no information will be copied or sent out.
